Middle School DECA Membership Registration Form
Thank you for registering your middle school members with Wisconsin DECA. Once you complete and submit this form, your students will be registered as middle school members. You will receive a confirmation email as soon as you click submit.Membership Payment: Shortly after, you will receive a formal invoice from Wisconsin DECA. Payment is due within 30 days of the invoice date.Once your membership is paid, Wisconsin DECA will mail your chapter a middle school flag and a middle school pin for each person you register. SECTION 1: Chapter Information
Please tell us more about your chapter and provide contact information for your primary advisor. Additional advisors can be registered under Section 2.
SECTION 2: Adding Individual Members Information
The next step to registering your middle school members is to create a list using this excel template to enter your members names, gender, affiliation (student or advisor), and grade. Once you have adding your members information to the template, you will be upload that excel document in the section below.
SECTION 3: Payment
As soon as you click submit on this form, you will receive a confirmation email that your chapter has been registered. Shortly after, you will receive an official invoice from Wisconsin DECA. Payment is due within 30 days of the invoice. Please include the invoice with your payment and send it to:
